Promotion Period: From Nov 18, 2024, to Dec 31, 2024 (inclusive of both start and end dates)
Eligibility: Participants who purchase designated Eye Level regular course of Dec 2024 or Jan 2025 during the promotion period of Nov 18 to Dec 31, 2024 are entitled to register for lucky draw chance.
Steps to Enrol:
Step 1: Enrolling in the Hong Kong Eye Level regular course for Dec 2024 or Jan 2025
Step 2: Fill out the required information on the lucky draw registration page and upload the receipt on or before Jan 1, 2025
Step 3 : Participants will receive their lucky draw number via email on/before Jan 7, 2025
The lucky draw results will be announced on Jan 17, 2025, on the official website (myeyelevel.com), Sing Tao Daily and The Standard. Winners will be notified via email and receive “Prize Collection Notification”.

Terms and Conditions:
1. Participants who purchase designated Eye Level regular course of Dec 2024 or Jan 2025 during the promotion period of Nov 18 to Dec 31, 2024 are entitled to register for lucky draw chance.
2. For each eligible tuition receipt submitted (for every four classes in the month), participants will receive one entry into the lucky draw. If a participant enrolls in courses for two consecutive full months, they will receive two entries into the draw.
3. Participants can accumulate entries for different subjects separately, with a maximum of 8 entries per participant. The subjects must be Eye Level regular courses, including: Eye Level Math, English, Chinese, Play Math, Writing, Grammar, and Summit of Math. 30-minute Top Up classes are not included.
4. The information provided by participants cannot be changed. Therefore, participants must verify their information before submission and ensure that a complete and clear photo of the valid tuition receipt is uploaded. All information is based on the records uploaded to the lucky draw registration page https://formfacade.com/sm/mIJUwvrNE .
5. Duplicate submissions of the same tuition receipt, incorrect, or incomplete information will be deemed invalid. The company reserves the right to cancel the participant's entry and/or prize eligibility.
6. If participants fail to provide the student's name, email, phone number, student ID or admission month when filling out the form, or if any information is unclear or incomplete, resulting in the company being unable to identify or contact the winner or verify their information, the entry will be deemed invalid.
7. The deadline for lucky draw registration is January 1, 2025, at 11:59:59 PM. The registration data and time for this promotional event will be based on the data collected by the company's server. The company is not responsible for any delays, losses, errors, or unidentifiable submissions due to computer, communication tools, internet issues, or any other problems beyond the company's reasonable control.
8. Participants will receive their lucky draw number via email on/before Jan 7, 2025.
9. The list of winners will be announced on January 17, 2025, on the Eye Level website, Sing Tao Daily, and The Standard. The lucky draw results and “Prize Collection Notification” will be emailed to the winners.
10. Winners must present the “Prize Collection Notification” issued by the company when claiming their prizes. The winner needs to provide relevant student identification documents for verification. If a winner fails to collect their prize by the deadline specified in the notification letter, they will be considered to have forfeited their right to the prize.
11. If a winner is unable to collect the prize in person within the designated time, they must provide written authorization for another person to collect it on their behalf. The proxy must present identification of the winner for verification.
12. All prizes are non-exchangeable for cash or other benefits. The company will not provide any proof or compensation for lost, damaged, or stolen prizes after collection.
13. The company is not the supplier of the grand, second, third, fourth and fifth prizes. Any disputes arising from the use or enjoyment of the prizes must be resolved between the winners and the relevant suppliers. Winners must comply with the terms and conditions of the suppliers when claiming their prizes. The company are not liable for any responsibility or legal obligations in this regard.
14. This lucky draw event is organized by Daekyo Hong Kong Co., Ltd.,. The personal data collected will only be used for the lucky draw and to verify participant eligibility. The company may publicly disclose and print the names and other relevant personal information of the winners for announcement purposes. Personal data will be deleted after the event concludes.
16. In case of any disputes, Daekyo Hong Kong Co., Ltd., reserves the final decision rights.
